You just need a template or what?  Is it for pirated stuff or your own films?

I'd like a template so that when it prints out I can slip it right in...

Photoshop is the best solution better then any program specially made for the purpose.

I know that NeroVision Express came with a template.

Go buy or pirate that.

You can download various templates here. (http://www.alliedvaughn.com/index.cfm?primarylinkID=404&secondarylinkID=456&tertiarylinkID=529&showcontentlinkID=529&showtertiarylinks=true)

When I was making mine, I just took a glance at the specific dimensions and then started from scratch in Photoshop - but you could also use the PDFs as a base layer and build upward from there.

If you want any help with design or anything, let me know. I'd be happy to help.
