1) Requiem for a Dream- it is the encompassment of what (imo) a title
sequence should be...cool titles and great music

Let's stick to title sequences for this thread:
1.) Seven (gets you in the mind of the John Doe long before he even shows)
2.) Psycho (great score to accompany the parallel bars that suggest duality)
3.) West Side Story (the end credits; so simple - graffitti on street signs - yet so effective)
4.) Casino (beautiful neon imagery)
5.) Clockers (it's use of crime scene photos is so powerful)

1 - Goodfellas - Simple, yet effective
2 - Fight Club - Riding the Brain Train
3 - Do the Right Thing - Rose Perez and Public Enemy: classic
4 - La Femme Nikita - love the Luc Besson intros
5 - Enter the Dragon - just love the Lalo Schrifin music and Jim walking badass

Godard's title sequences were usually great. My favorites:
-Vivre Sa Vie
-Band of Outsiders
...and the number one Godard title sequence: The original one (as seen on the Criterion DVD) from Contempt.
Also, anything Woody Allen, with his great signature titles.
Fassbinder titles- I'm thinking of The Marriage of Maria Braun in particular- tend to be done in very nice fonts.
I second The Royal Tenenbaums as great title sequence, which someone else mentioned above.
the opening credits in Irreversible is embedded into my brain.
