liability waiver--help?

this is my first time shooting as a director and a producer, and all this paperwork crap is very exotic to me.  this morning I was told that I'll need to submit a liability waiver form to the bar manager before shooting there.  I have no idea what a liability waiver form looks like.  has anyone written one of those before?  can you please post them words here?

that was a while ago and I just made up some legal sounding paper.
no liability insurance, this is like a tiny short project (with a script no one bothered to read or bothered to tell me how bad it was) that I'm just doing when my stuntmen friends are free.
